In the event of any accident, illness or injury, I agree on behalf of myself, my child's other parent if known or living, the children named above, or our heirs, successors, and assigns to hold harmless and defend the Archdiocese of Galveston-Houston, the sponsoring parish, its pastor, youth
ministry leader, principal, other agents, employees or other representatives associated with the event from any and all injuries, losses
or claims arising out of my child’s participation in the event.

I understand that promotional pictures and videos may be taken during Faith Formation classes/events. I give permission for my child (named above) to be used in church promotional materials.

STUDENT DROP OFF/DISMISSAL PROCEDURES
1.Students are required to sign in upon arriving for EDGE.
2.If a student needs to leave early, the youth minister needs to be informed by a parent/guardian
3.Students are not permitted to leave until we receive confirmation their rides have arrived atSt. Vincent de Paul (either coming inside the facility, text message to student, etc).

ALLOWANCE ON GROUNDS
Parents or other adults are not allowed inside any classroom building on EDGE outside of the drop-off and pick-up times, unless they are Certified by the Archdiocesan Safe Environment Training (Virtues/ Safe Haven)

ATTENDANCE
Regular attendance at EDGE classes are very important. Please make every effort to bring your child each week at the proper time.
Students receiving sacramental preparation must attend EDGE nights alongside of their sacramental prep classes. If they exceed 3 absences in one year, they may be required to do make-up work in order to complete their preparation requirements.

ABUSE PREVENTION CURRICULUM
Cardinal DiNardo has directed that all archdiocesan parishes participate in Circle of Grace, a safe environment education program for all children in Kindergarten-12th grade. Its primary focus is to help children and their parents develop the knowledge and skills needed to keep children and young people safe and protected from abuse. Circle of Grace provides three age appropriate lessons each Fall to children, which will be combined into one EDGE. Parents are strongly encouraged to preview the lessons, which will be emailed out prior to class.

CLASSROOM RULES & CONTROL
To maintain a beneficial learning environment for all students, we ask that all children follow the basic rule of respect for their catechists and classmates. If a student manifests a difficulty showing respect, the parents will be contacted, so that a solution can be reached.

PERMISSIONS
By registering your child(ren) for faith formation you give permission to the parish to provide its catechists with access to your child(ren) for the purpose of catechesis. Depending on the circumstances and the program, this access may be face-to-face in a parish facility, or may be on a virtual platform approved and overseen by the parish, or both. All catechists have been cleared by the Archdiocesan Office of Child and Youth Protection to work with minors and have completed all required safe environment training

I am the parent or legal guardian of the child or children named above. I have read the faith formation policies & guidelines or have had them read to me. I fully understand and accept these policies & guidelines knowingly, freely & willingly.
